Information For New Patients
INFORMATION FOR NEW PATIENTS
If you are a new patient, please click on the link below and log into the patient portal to complete all the New Patient questionnaires. New patients should have received an email with their patient portal login information/link when you make your appointment. If you did not receive the email link you can contact us via email at md@unitedderm.com or fill out a website inquiry to the right of the screen.

Insurance Information
United Dermatology Associates accepts a wide variety of insurances and payments. Please check the following list to see if we are on your insurance plan.If you have a HMO plan or a plan that requires a referral, it is your responsibility to obtain the referral from your PCP before the initial visit. If we have not received the referral, we will be more than happy to see you as a private pay patient. If your insurance provider is not on the below list, please call or E-mail our office.
